Magnetic resonance imaging evaluation of oblique injuries: Introduction of the deadrise sign

William Welton, Nicholas Hanna, Vincent Key, Brian M. Everist, Jamie Crist ยท Journal of Clinical Imaging Science ยท 2026

Oblique injuries, which occur when forceful trunk rotation tears the internal oblique muscle from the ribs, are common in sports and cause significant missed playing time. This study reviewed MRI images of oblique injuries at a single institution and introduced the "deadrise sign" as a diagnostic finding, along with guidance on MRI protocols and treatment approaches.
Takeaway: Use MRI and the deadrise sign to properly diagnose oblique injuries when athletes report sudden lateral abdominal pain with trunk rotation.
Abstract (source)

Oblique injuries are a common sports-related injury to the lateral abdominal wall, most commonly caused by forceful trunk rotation, which tears the internal oblique from the undersurface of the lower ribs. Despite increased awareness and prevention efforts, these injuries remain a significant contributor to missed playing time. A single institution retrospective cohort review of oblique injury magnetic resonance images (MRIs) was performed, followed by a comprehensive review including relevant anatomy, grading considerations, and
